📌 What Is the Wells’ Score?

The Wells’ Score is a clinical prediction rule that helps estimate the probability of DVT or PE based on a combination of patient symptoms, history, and clinical findings. Developed by Dr. Philip Wells and colleagues, the score has become a standard tool in emergency medicine and primary care.

There are two versions:

  • Wells’ Score for DVT includes criteria such as active cancer, recent immobilization, localized tenderness, and swelling.
  • Wells’ Score for PE includes factors like signs of DVT, heart rate >100 bpm, recent surgery or immobilization, and previous DVT or PE.

Each criterion carries a point value. The total score stratifies the patient’s likelihood of having a DVT or PE into “likely” or “unlikely” categories — guiding further diagnostic testing such as D-dimer or imaging.

The Pulmonary Embolism Rule-out Criteria (PERC) is another tool used to identify patients at low risk for PE, potentially reducing unnecessary imaging. The PERC rule includes eight criteria: age <50 years, heart rate <100 bpm, oxygen saturation ≥95%, no hemoptysis, no hormone use, no prior DVT or PE, no unilateral leg swelling, and no recent surgery or trauma requiring hospitalization. If a patient meets all these criteria, the likelihood of PE is considered low, and further testing may not be necessary.
